Since its inception in 2004, the Supportive Housing Leadership Forum has assembled “systems builders” from around the country for a two-day meeting to:
- Share examples of how multiple sectors are working together in innovative and efficient ways to leverage the resources needed to create and sustain the operation of supportive housing.
- Offer collective experience to inform public officials about how selected states and localities are financing the development and long term operation of supportive housing; share lessons learned and the common features of these approaches, including typical funding sources and policy reform strategies.
- Showcase emerging techniques intended to ensure that people with chronic health conditions who also are frequent users of multiple institutional settings such as hospitals, jails, prisons, and treatment centers are discharged into housing, including supportive housing, not homelessness.
